Eligibility

• Entrant (“You”) must be 18 years or older
• Void where prohibited or restricted by law
• Entrants must be able to travel to the office of Preferred Eye Care (“Company”) during normal operating hours to collect the prize.

• Entrant is not required to be a patient or client of the Company.

Sponsorship

• The Company is the sole provider of the prizes; this sweepstakes is not sponsored, endorsed, or administered by any third party

Agreements

• By entering into the sweepstakes, the Entrant (“You”) agrees to abide by the Company’s Official Rules and decisions, which are fully and unconditionally binding in all aspects.
• The Company reserves the right to refuse or disqualify any entry at any time at their sole discretion.
• Entrant agrees to accept the decisions of The Company as final and binding as it relates to the content of the Sweepstakes.
• Entrant agrees to be contacted by our office via phone, email, social or other media.
• Entrant agrees to have their name or username mentioned or listed on the Company’s website and social media pages, including but not limited to: Instagram, Facebook, X, TikTok.
• Entrant/winner agrees to appear and have their photo posted onto Company website and social media pages.
• Entrant understands the Company may change or revise rules during the sweepstakes period in order to maintain fairness of the event for all entrants.
• Entrant is bound by these terms and any additional rules or eligibility requirements as determined by the Company
• Entrant is participating on a voluntary basis and understand they will not be compensated in any way.

Entry Period

• Now until December 31, 2025
• Drawing will occur the first weekend of the year 2026.
• Winner will be announced the following Monday.

How to Enter

• Fraudulent methods of entry or circumventing the rules may result in the Company invalidating entries and removing participants.
• (1 ENTRY / POST) While following the social media account, “Like” and “Repost” any sweepstakes-related posts on social media. Both actions must be performed.
• (1 ENTRY / PLATFORM) Tag us in a social media post wearing the eyewear you received from us.
  • Entrants may inquire for the Company’s social handle if it is not posted or if the Company does not have one for that particular media.
  • The Company may create a social profile on other platforms if requested by entrant provided it serves to promote the Company’s business.
• (1 ENTRY / REFERRAL) Referring a patient.
  • Referred patient must report entrant as the referrer at the time of the exam. It is the responsibility of the referred and referrer to report the referral.
    • Longstanding and established patients of our office will not count as a referral.
  • Referred patient must not be a dependent or immediate family member of the referrer
  • Referred patient must be seen for an eye exam at our office during the sweepstakes entry period.
  • Referral of a family counts as 1 entry
    • g. Referrer does not receive 4 entries for a family of four, whether seen on the same day or different days.
  • The Company reserves the right to decide if a referral counts as an entry and may invalidate an entry depending on surrounding circumstances.

• (1 ENTRY) Leave a 5-star review on our Google page.
• Entrants may submit multiple entries.
  • The Company will submit entries based on name and usernames. If the Company is able to ascertain a common identity, entries will be collected under common identity.
  • The Company will not report or acknowledge entries at the time of receiving them.
  • Entrants may request a tally of total entries at any time. Entrant must be able to provide all names or usernames used in submitting an entry for verification.
  • Entrants may report or request acknowledgement of entry, but the Company is not required to respond.
  • The Company reserves the right to accept or refuse the entry.
  • The Company may, but is not required to, cite reason for refusal of entry.

• Winner will be allowed to select from one of the following
  • Pair of sunglasses from select available styles. Valued up to $200.
    • Prescription lens options available for a discounted price
    • Selection is dependent on what is available at the time the winner is drawn.
  • Prescription lenses – winner must have valid prescription. Exam or refraction is not included with the prize.
    • Prescription glasses
      • Frame value from our office up to $150 with polycarbonate, standard anti-reflective lenses
        • Additional lens treatments available for reduced rate.
      • Winner’s may opt to use own frame and fill with prescription lenses up to $200 value with select add-ons.
    • 6-month supply Daily, Weekly, or Bi-weekly replacement contact lenses
    • 1-year supply of Monthly Replacement contact lenses
  • $200 Visa Gift Card.
• Prizes are not covered under warranty
• Prizes are not returnable, refundable, or exchangeable
• Winner may transfer selection of prize to another individual
  • Winner will need to provide evidence (photo ID and signature) of prize transferal or present with individual to office when selecting the prize.

Odds

• Total number of eligible entries received determines the odds of winning

Selection and Notification of Winner

• Only one winner will be selected via random drawing or roulette
• Winner will be selected on the first weekend of January 2026
• Winner will be notified via social media
• If the winner violates any of the official rules, the winner will be disqualified, and all privileges as winner will be immediately terminated.

Rights granted by you

• By entering into the sweepstakes, you understand and agree that the Company has the right to print, publish, broadcast, and use in any media known now or hereafter developed, and without limitation, your name, photo, image, statements about the Company’s sweepstakes, and your biographical information for news, publicity, advertising, promotional purposes, trade, information, and public relations without any further notice, review, consent, compensation or remuneration.

Terms and Conditions

• In its sole discretion, the Company reserves the right to modify, suspend, cancel, or terminate any and all Sweepstakes should non-authorized human intervention, a bug or virus, fraud, or other causes beyond the Company’s control, impact or corrupt the security, fairness, proper conduct, or administration of Sweepstakes or Contests. The Company, in the event of any of the above issues, may determine the Winner based on all eligible entries received prior to and/or after (if appropriate) the action taken by the Company.
• Individuals who tamper with or attempt to tamper with the operation or entry process of the Sweepstakes or website or violates these Terms & Conditions will be disqualified by the Company in its sole discretion.
• In its sole discretion, the Company has the right to maintain the integrity of Sweepstakes, to void votes for any reason, including, but not limited to: Multiple entries from the same computer beyond the number allowed by the Sweepstakes rules; or the use of bots, macros, scripts, or other technical means for entering. Attempts by any entrant to deliberately damage any website or undermine the legitimate operation of any or all Sweepstakes may be a violation of criminal and civil laws. If any such attempt is made, the Company reserves the right to seek damages to the fullest extent permitted by law.

Limitation of Liability:

• Your entry into the Company’s Sweepstakes or constitutes Your agreement to release and hold harmless the Company and its subsidiaries, representatives, affiliates, partners, advertising and promotion agencies, successors, agents, assigns, directors, employees, and officers against and from any and all claims, liability, illness, injury, death, litigation, loss, or damages that may occur, directly or indirectly from participation in the Sweepstakes or Contest and/or the
  1. Winner accepting, possessing, using, or misusing of any awarded prize or any portion thereof;
  2. any type of technical failure;
  3. the unavailability or inaccessibility of any transmissions, phone, or Internet service;
  4. unauthorized intervention in any part of the entry process or the Promotion;
  5. electronic error or human error in the Promotion administration or the processing of entries.

Disputes:

• THE COMPANY’S SWEEPSTAKES IS GOVERNED BY THE LAWS OF THE UNITED STATES OF AMERICA AND TEXAS, WITHOUT RESPECT TO CONFLICT OF LAW DOCTRINES. Under no circumstances in any such dispute shall the participant be permitted to obtain awards for, and hereby waives all rights to, punitive, incidental, or consequential damages, including reasonable attorney’s fees, other than participant’s actual out-of-pocket expenses (i.e. costs associated with entering Company’s Sweepstakes.) The participant waives all rights to have damages multiplied or increased.

Winners List:

• You may obtain a copy of the Winner’s name(s) or a copy of these Official Rules, by sending your request via mail with a stamped, self-addressed envelope to: 2454 Cypress Creek Parkway, Houston, TX 77068.

Social Media Channel-Specific Rules:

• The Company’s Sweepstakes hosted by the Preferred Eye Care is in no way sponsored, endorsed, administered by, or associated with Facebook, Instagram, X (formerly Twitter), or any other social media platform

By participating in the Company’s Sweepstakes, You, the Entrant, have affirmatively reviewed, accepted, and agreed to all of the Official Rules.
